Universal Benefits Application
The USDA wants everyone to know the following information: States, Indian Tribal Organizations, and territories can provide families with about $120 per eligible school-age child to buy groceries in the summer months at authorized retailers. This program is also known as Summer Electronic Benefits Transfer or Summer EBT in some locations. This form may not be necessary depending on your individual circumstances. Please check with the business manager in the office find out if you need to complete this form. Once the form is completed you may bring it into the office or email it to businessmanager@treeoflifeschool.net. The office will notify you in writing within two weeks of your child’s eligibility.
